EMPOWERING FAMILIES FOR SUCCESS
What is EFS?
Empowering Families for Success (EFS) is a Child & Family
Services program that provides flexible support to families with issues and
concerns related to mental health. Most of the services are provided
in a family's home at their convenience.
Since families know best what will and will not work for them, EFS
acts as a partner in developing an individualized treatment plan.
EFS treats families with a variety of tools including Family
Intervention, The Wrap Around Process, Youth Support Workers, Life
Skills Coaches, Flexibles Support Funds, and groups with parents,
children and adolescents.
Typically a Care Manager and/or Life Skills Coach work with each
family or teen. When possible, families are teamed with staff who
are familiar with their language and cultural background.
Who are the EFS Clients?
The EFS Program works with families who have youth
ages 3-18 with acute mental health needs that require
more than traditional treatment.
How are Referrals Made?
Families are referred to EFS by the Department of Mental
Health to help reduce psychiatric in-patient hospitalizations,
and/or residential treatment and prepare teens for independent
living.
